How to Use This Form

The registration form functions as a registration request: we process registration forms beginning Tuesday, December 3 and students receive an email confirmation once their form has been processed.

Students are encouraged to read the course catalog before filling out the registration form. If someone in the student’s life helps with making financial decisions, PHAME recommends filling out the registration form with that person.

Any student registering for in-person classes must comply with PHAME’s Wellness & Covid Policies.

Winter 2025 Deadlines:

For priority registration, submit this form by Tuesday, December 3, 9 AM. The last day to submit this form is Monday, January 6.

New to PHAME?

This form is for current PHAME students. If you have not taken classes at PHAME before, the first step is to schedule your prospective student interview. 



Registration Request Form

 

You will not receive a confirmation email after submitting your registration form. Instead, you will see a message appear that reads “You have successfully submitted your Registration Form.”


Registration & Withdrawal Policies

By filling out the registration request form, you give PHAME permission to register you for the classes you have indicated on your form and you acknowledge that this is a binding agreement subject to the following PHAME policies.

Classes:
Tuition for each class includes a $10 non-refundable registration fee. The last day to drop a class without paying an additional fee is the Monday before the first week of the term (if Monday is a PHAME holiday, the deadline moves to Tuesday at noon). For classes dropped during the first or second week of the term, or in the week before the term begins, students will be responsible for paying 25% of tuition owed or $10, whichever is greater. For classes dropped after the second week of the term, there are no refunds and students will be responsible for 100% of tuition owed.

The last day to register or add a class without incurring a late fee is the Monday before the first week of the term (if Monday is a PHAME holiday, the deadline moves to Tuesday at noon). After that date, registration must be done by calling Alex at (503) 764-9718 ext. 18 and will incur a $10 per class late registration fee.

Please note:
Unexpected circumstances may cause PHAME to cancel classes prior to the start of term for any reason. Any tuition already paid will be credited to students’ accounts or refunded if requested.
